Your First Visit
Thank you for choosing Wertsch Vision Associates for your eye care needs. We are 
happy to welcome you to our office. There are a few things we would like you to know before your first visit, so that you can come prepared.
What to bring to your first visit
A new patient form will be mailed to you when you schedule your first appointment. You may either bring it with you, or mail it to our office prior to your visit.
If you have insurance, please be sure to bring your cards with you. Please have both medical and vision cards, as your exam may be able to be submitted to your medical insurance (please see FAQ: Is it medical or vision?) Please bring a list of any medications or supplements you are currently taking, as well as a history of allergies.
We also ask that you bring any glasses, contact lenses, or vision aids that you may have.
Your doctor will also want to know the name and address of your primary physician.
If you are coming for a Low Vision Exam
If you are coming for a Low Vision Exam, please bring any low vision devices that you may have. It is also important to know that due to the extent of the low vision exam, the length of the exam will be 2-2 1/2 hours long. Please make necessary transportation arrangements and inform them of the length of the exam.
